Abstract

Objective: To investigate the information an 'ideal' discharge medication list should contain. Design and methods: An observational study was conducted. Through e-mail several healthcare providers, such as (hospital) pharmacists and physicians, were invited to a conference regarding discharge prescriptions. They were also invited to fill in a questionnaire on the internet (September-November 2008). The main topics in the questionnaire were the desired information on a discharge prescription list regarding patient, hospital and medication. Frequencies were calculated on each question. Remarks given by the respondents were grouped and analyzed qualitatively. During the conference the results of the questionnaire were presented. Subsequently, the results were discussed with participants of the Amsterdam region. Results: The questionnaire was filled out by 142 persons (88%). The main respondents were (hospital) pharmacists (67%). The respondents desired a variety of information parameters, such as allergies, contact information and reasons for changes in the pharmacotherapy. Respondents wished to receive a standardized discharge medication list that gives a complete overview of the medication (not only the medication that has to be dispensed). This discharge medication list should be sent to the community pharmacist and the general practitioner of the patient. After discussion of the results during the conference further nuances were made to the 'ideal' discharge medication list. Conclusion: With the help of healthcare providers a minimal set of required data was obtained for an 'ideal' discharge medication list.
